Procedure for requesting disclosure or other action concerning personal data held by the SRI Group

  1. Whom to contact with requests for disclosure or other action concerning personal data held by the SRI Group
    For requests for disclosure or other action concerning personal data held by the SRI Group, please submit your request to the location where you registered your information.
    If you are unsure of whom to contact, please attach the required documentation to the specified form and send it by regular mail addressed to the administration department indicated below.
    The SRI Group cannot accept requests for disclosure or other action except by regular mail. We apologize for any inconvenience and thank you for your understanding.

  2. Procedure concerning disclosure of personal data held by the SRI Group or other action
    For requests for disclosure of personal data or other action, please complete all of the items indicated on the request form, and mail the form together with documentation for verification of your identity to the Inquiries Office.
    Requests for disclosure of personal data or other action also can be made by an appointed agent. In such instances, however, please enclose documentation that will verify your agent together with the above-mentioned required documentation and mail the forms to the Inquiries Office.

    Documentation for verification of your identity
    One copy of your driver's license, passport, health insurance card, alien registration certificate or Basic Resident Register card showing a name and address identical to the name and address of the individual who is requesting the disclosure of personal data or other action.

    Documentation for proof of agent
    One copy of both your Authorization of Agent (signature and seal of the individual in question) and Certificate of Seal Impression with the same impression as the seal of the individual who signed and attached a seal to the Authorization of Agent
    Documentation to verify the agent
    Other documentation to verify identification of the individual as needed
  3. Fees
    When submitting requests for disclosure of personal data and requests for notification of purpose of use, there is a fee of 500 yen per application. Please enclose postage stamps with a value of 500 yen with your request.
    Depending on the content of a request for disclosure of personal data or other action, SRI will require payment of the actual expenses incurred for its search. In such instances SRI will send you an estimate beforehand.
    There are no fees for requests concerning corrections, additions, deletions, terminations of use, eliminations or termination of provision to third parties of personal data contents.
  4. Procedure for responding to requests for disclosure of personal data or other action
    For requests for disclosure of personal data and requests for notification of purpose of use, SRI will submit its response in writing to the address shown on the applicant's request form. For requests concerning corrections, additions, deletions, terminations of use, eliminations or termination of provision to third parties of personal data contents, as a rule SRI will respond by telephone or e-mail.
